The short version: Most U.S. sperm banks accept men roughly 18–39, at least 5'8" tall, in good health, non-smokers, with some higher education, able to pass semen quality, medical, and genetic screening. Acceptance is competitive -- fewer than 5% of applicants are accepted at top banks.

How to apply
